Getting Started

Welcome to Dr. Hall’s New Patient Center!  Scheduling your first appointment with Dr. Hall is simple.  Here you will find all the necessary new patient intake forms and learn what to expect on your first consult.  If you have any questions that aren’t answered here or in other sections of the site, please call our office Monday through Friday at (805) 687-0533 and the front desk staff will be happy to assist you.

New Patient Intake Forms

The first step is filling out the intake forms ahead of time.  When doing this, it is best to set aside 30-45 minutes where you will be free of distractions.  Our ability to draw effective conclusions about your state of health and how to optimize it’s improvement depends largely on the accuracy of the information in which you provide, including symptoms that you may consider minor.  Health issues may be influenced by many factors; therefore, it is important that you carefully consider the questions asked in this form as well as those posed by the doctor during your consultation.  This will assist our goal to provide you with an optimal plan of health care, enhance our efficiency, and will provide effective use of your scheduled time.

Once you have completed the forms, please return them to us.  You may do this via email  (office@santabarbarawellnessforlife.com), fax (805.687.0620) or drop them by the office Monday through Friday.  Because the doctor needs ample time to evaluate your case, we ask that you return the completed forms a minimum of 2 days prior to your visit.

Initial Consultation

Your initial consult will take 60-90 minutes.  Dr. Hall will begin with a thorough review of your medical history and intake forms, where he can gather any additional information that pertains to your primary health complaints.  This session will help establish your current state of health and determine what your immediate and long-term health goals are.  This is also the time for you to ask the doctor any questions you may have.

Following your history, Dr. Hall will perform a physical and nutritional exam.  Based on the information obtained from your detailed history and exams, the doctor may recommend special lab tests to help arrive at a diagnosis.  This initial visit will lay the foundation to create a personalized plan that will ultimately lead you to optimal health.

On your first follow up visit, Dr. Hall will go over the exam findings and any lab results and explain how they impact your health.  At this point we will have a clear picture of what needs to be addressed and you will be provided with a clear and direct clinical management plan to optimize results.

Chiropractic treatment, specific dietary changes, nutrient supplementation, physiotherapy, exercises and stress-reduction techniques may all be components of your healthcare plan.  As you can see, after just 2 visits you’ll be well on your way to regaining your health and improving your quality of life!

Follow-up Visits

The number and frequency of follow-up visits are determined by the patient’s condition.  These usually last fifteen to twenty minutes.  Dr. Hall will review the current treatment plan and address any concerns to ensure progress continues in the right direction.  As symptoms improve, adjustments are made to your wellness plan, ultimately leading to maintaining an optimal state of health.  Missing or delaying follow-up visits will prolong the process and make it more difficult to achieve the desired results.

Phone Consults

Dr. Hall helps patients across the US with their health concerns.  These consults are treated the same as office visits, whereby the intake forms, patient history and lab work will be used to create a wellness plan.  In this case, the plan may include co-management with a local chiropractor or other healthcare provider.  If you would like to schedule an appointment but are unable to visit the clinic, please call the office to see if this option will work for you.

Insurance

Dr. Hall accepts most major forms of insurance.  Reimbursement from your insurance company is determined by your specific plan and annual deductible.  Additionally, many insurance plans impose caps on the amount that they pay per office visit.  We will copy your insurance information on your first visit to verify benefits, but recommend that you also call your insurance company ahead of time to check the specifics of your plan and determine if your deductible has been met.  You will pay for services rendered by Dr. Hall at the time of your visit until it has been determined what your insurance company will reimburse.  You may pay by check, credit card or cash.

Lab Work and Special Tests

Our office does not bill insurance for lab work or special tests.  However, most of the labs we work with can and will bill insurance on your behalf.  If you opt to have the lab bill your insurance, they will likely require a co-pay upon submission of the test.   Any amount not covered by your insurance will then be your responsibility.  Alternatively, many labs offer a discounted fee schedule for patients who choose to pre-pay out of pocket.  Often times these discounts are significant, in which case our office recommends utilizing the pre-pay option to ensure the lowest rates.  Additionally, some special studies focus on prevention, and your insurance company may determine them to not be “medically necessary”, i.e. not a matter of life and death.  Please contact your insurance company to determine if the recommended lab work will be covered prior to billing.

Location

Our office is located at 3888 State st., Ste. #203, near the corner of State st. and La Cumbre.

We are on the second floor of the building above Chuck’s Steakhouse and My Gym.  There are plenty parking spaces in the lot in the back, which can be accessed from either State st. or La Cumbre.  We do have an elevator if necessary, which can be easy to miss.  If you need any assistance finding the office or making your way up from the car, we are happy to help, please just give us a call.
